What Will Happen On My First Visit?

Please assist us at the time of your initial visit to the office by:

• Bringing your referral slip and X-rays from your referring dentist.
• Listing down the medications you are currently taking.
• Bringing any forms or insurance cards with you, if you have dental insurance, to the appointment.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Notify the office if you have a medical condition or concern prior to a dental or periodontal procedure and surgery (e.g., artificial heart valves or joints, heart murmurs requiring premedication, severe diabetes, or hypertension, cancer treatment or taking blood thinners of any kind).

PLEASE WEAR A MASK WHEN YOU COME TO YOUR VISIT.

• Answering a COVID-19 questionnaire that our staff will provide you with at time of checking in.
• Your temperature will be taken and recorded at every visit.
